Evan Hathaway DDS PA is located at 1217 E. Iron Eagle Drive, in the Rocky Mountain Business Park. Coming from State Street/Hwy 44, go north on Edgewood and Iron Eagle Drive is immediately on your left side.
Financial Arrangements:
Our goal is to not let expense prevent you from benefiting from the quality of care you desire and need. Our practice will assist you by filing your insurance claims. We will maximize your insurance benefits and attempt to estimate any remaining balance. To facilitate our goal, we have developed the following financial options:
1. Cash or Check:
If your treatment plan is over $1000.00 and you pay for your treatment in full, in advance or on the day of your appointment we offer a 5% bookkeeping courtesy.
2. Payment Plans:
A) Payment in 90 days: Three monthly payments by pre-authorizing your checking account (debit card) or an existing credit card. Your account will be billed for three payments on the dates specified by the patient. The first payment is due on the date of service.
B) Care Credit: Care Credit is a line of credit that can be established privately on line and we offer up to 12 months interest free (OAC). Care Credit offers other plans up to 60 months with competitive interest rates. Care Credit allows us to help make the smile you’ve always wanted affordable.
For your convenience we also accept VISA, MasterCard, American Express and Discover.
We are providers/in network for the following insurance companies:
Delta Dental Premier, Delta Dental PPO, Blue Cross Traditional, Blue Cross PPO, Blue Shield of Idaho, United Concordia
*If for any reason the estimated amount is not paid by your insurance company, it becomes your obligation.

Our providers are not specialists in family, crowns, gum disease, sedation, prosthodontics, orthodontics,
endodontics, cosmetic, whitening, implants or non-specialty interest areas that require specific educational
training to advertise this service.
